Jon San Martin

Jon San Martin is an experienced Registered Nurse with a strong interest in the intersection of ageing, dementia, and palliative care. Jon’s clinical experience spans residential aged care and specialist palliative care services in both hospital and community settings.
He is passionate about improving access to palliative care through workforce development, mentoring, and education. Jon contributes to strengthening workforce capability and enhancing the integration of palliative care across primary, home, and aged care settings through his involvement in the national implementation of two Palliative Care Projects funded by the Australian Government: The Advance Project and the End of Life Direction for Aged Care (ELDAC) Project, alongside teaching in undergraduate and postgraduate nursing programs.
Jon currently works as the Palliative Care Clinical Nurse Consultant at HammondCare and as an Education Research Fellow (Palliative Care) at the University of Technology Sydney.
