Moana Jagasia
- Moana is a Consultant at NWC Longevity Practice with close to 10 years of professional
and academic experience in Singapore’s government and non-profit sectors. She has
coordinated and run initiatives in chronic disease management, preventive health and
primary care to improve health outcomes in community-based settings in collaboration
with academic and government agency partners. - These include managing operations of a primary care clinic modelled after the Person-
Centred Medical Home (PCMH) for frail older persons as well as reviewing service
models in terms of scope, manpower and resourcing for community-based services for
older persons and their caregivers at Tsao Foundation. - Through her work, Moana has acquired skills in programme development and research coordination at the individual as well as population health level and has extensive experience working with partners across disabilities, ageing and mental health.
- Moana has a Master in Public Policy degree from the Lee Kuan Yew School of Public
Policy at the National University of Singapore and a Bachelor of Arts degree from Duke
University. She also has the WSQ Advance Certificate in Learning and Performance
(ACLP) for workplace training and is certified in Implementing Interventions in Health
and Social Care (social prescribing for Healthier SG).
